Hmm. You were supposed to have received the Supplemental Application Form as an attachment to the email you received when you sent in your on-line application form.
This page is here in case you didn't get it, or if you mistakenly trashed it. Please ignore this if you have successfully completed your Supplemental Application. If not, instructions are below.
Instructions for the Supplemental Application Form
We now need some additional printed materials for your application folder, as well as your application fee. These two items are required before we can schedule you for an audition or interview. Page 4 is a credit card authorization for your application fee.
Click the link below to get your Supplemental Application form. You can fill it out on-screen, but to do so requires the latest version of Adobe Acrobat Reader. You can click the Adobe icon below to get it.
Print out two copies. You will need one for yourself, and one to send to us along with your application fee. When you are done, use your browser's "back" button to return here.
