In these examples, students have worked in different ways to clarify their efforts through the process of revision. This allows me to see precisely what a student was thinking, and what s/he actually did, in responding to my comments and suggestions.
The color-coded scheme, use of arrows (boxes and other geometrical shapes can be handy, too), and comments (as notes of explanation) all help me to see a student's thought process and manner of working, as well as the actual changes made in revision.
If you can submit something generated entirely in your computer, it is easy to make a pdf. But it should also be easy enough, if your final work is in hard copy, to scan it into a pdf document and post that to your portfolio.
