Peabody/Hopkins Chorus and Peabody Singers Excuse Request Form
An excuse request form must be submitted within one week following every rehearsal for which you are absent or late. You must submit a note from your doctor if you miss more than one rehearsal in a row for medical reasons. Please consult your Peabody Student Handbook if you have any questions about ensemble attendance policies.
