Disability Resources

General Information

Student Affairs Office Suite, Unger Lounge
410-234-4538

The Peabody Institute of the Johns Hopkins University values diversity and inclusion. We are committed to a climate of mutual respect and civility among members of our community. Peabody recognizes that disability is an aspect of diversity that adds richness to our community. To this end, Disability Resources in the Student Affairs Office collaborates with students, faculty, and staff to create learning environments that are usable, equitable, inclusive and welcoming. However, there may be times when there are aspects of the instruction or design of courses that result in barriers to inclusion or to accurate assessment of achievement. Students with disabilities who anticipate barriers to full participation in courses and/or campus activities are encouraged to contact Katsura Kurita, associate dean for student affairs and disability resources coordinator, in the Student Affairs Office.

In order to ensure a smooth transition to secure reasonable accommodations, students may sign up for services by completing the forms listed below. Students may also call to schedule an appointment with Katsura Kurita, associate dean for student affairs and disability resources coordinator, at 410-234-4538. Students are encouraged to identify themselves as early as possible since some accommodations may take time to put in place. Even if a student does not anticipate needing any accommodations at this time, we encourage students to bring or send in documentation and contact the disability resources coordinator. In this way, this office will be ready to move with expediency on a request that may surface at a later date.

Documentation

The guidelines about documentation of a disability have been established by the Johns Hopkins University Office of ADA Compliance and Disability Services, and are as follows:

  • Documentation must be completed and signed by a qualified professional with relevant expertise in the disability in question.
  • Documentation must describe the current functional limitations resulting from a disability and should include any relevant evaluations or diagnostic test scores.
  • Documentation must include a summary of the history of the disability.
  • Documentation should include a list of recommended accommodations that will address the functional limitations of the disability.
  • Documentation must refer to a recent diagnostic test or medical review, based on the type of disability.

More specific information can be found in the documents below (created by JHU Disability Services) or by visiting the website of the Office of ADA Compliance and Disability Services.
