You can download and fill some of the forms required for certain legal procedures. Do not submit any forms without prior consultation with your DSO (International Student Advisor). If you have any questions, schedule an appointment with the office of International Student Affairs.

Reduced Enrollment Request Form
A reduced course load is permitted with very specific circumstances. If you need to request reduced enrollment, please speak with the International Student Advisor.

AR-11 Change of Address
Purpose of Form: To report the change of address of an alien in the United States.

USCIS Form G-1145, E-Notification of Application/Petition Acceptance

Submitting this form allows an Applicant or Petitioner who has filed an application/petition at one of the three USCIS Lockbox facilities to receive electronic notification (e-Notification) that their application has been accepted.

Social Security Number Request
If you will be working in the United States, you will need to request a Social Security Number through the local Social Security Office.

Request for Letter of Invitation for Family Members
This is an internal form (used for the purposes of the Peabody Conservatory's Office of International Student Affairs). It will help us expedite your request for an official letter of invitation for members of your family willing to visit you in the U.S. This is not a letter of invitation. The actual letter of invitation is issued by the International Student Affairs office. This form is not a legal document and does not have any legal standing with any of the U.S. consular services.

International Students Important Information

Change of Status
I came on a B non-immigrant visa, but I am going to be a full-time student. Can I change my visa status to F, J or M?

A nonimmigrant who wishes to enter the United States and visit schools with the intent of possible attendance should declare his or her intention and have Prospective Student noted on the Form I-94. Otherwise, if a B-1/2 nonimmigrant wishes to change to an F-1 or M-1, he or she must provide evidence to explain the change of his or her primary purpose for entering the United States.

International Students—Grace Period
Information regarding the additional period of time that students are allowed to remain in the United States upon completion of their course of study/training.

I-94 Card
Provides basic explanation of the I-94 Card

Employment
Contains general guidelines on employment for nonimmigrant students. Off-campus employment must be approved by the Department of Homeland Security on a case by case basis. Students on non-immigrant F and J visas MUST ALWAYS consult the International Student Adviser (DSO) BEFORE seeking off-campus employment.
