The purpose of this form is to enable Johns Hopkins University Campus Ministries to serve you by knowing your religious affiliation. This information will only be released to Johns Hopkins Campus Ministries for the use of the University Chaplain along with the appropriate and recognized Campus Ministries and clergy. The completion of this form is optional.
