Disability Support Services

General Information

The Peabody Institute, under the provisions of the Americans with Disabilities Act of 1990 and Section 504 of the Rehabilitation Act, provides services to qualified students with documented disabilities. Disability Support Services at the Peabody Conservatory is coordinated through the Office of Student Affairs.

Federal law defines disability as a physical or mental impairment that substantially limits one or more major life activities. It is important to note that a condition in and of itself does not necessarily constitute a disability. The degree of impairment must be significant enough to substantially limit one or more major life activities (i.e., walking, seeing, hearing, speaking, breathing, learning, working, or taking care of oneself).

To determine eligibility, we require current and comprehensive documentation of the student's disorder. The following information is provided to assist Peabody students who wish to obtain disability-based accommodations.

To receive accommodations, students must:

  1. Submit recent and comprehensive documentation about the current functional impact of the disability in a university setting;

  2. Download and complete the request form below; and

  3. Submit the request form and required documentation to the Office of Student Affairs.

Once the Student Affairs Office receives the required materials, they will be submitted to the Director of ADA Compliance and Disability Services for the Johns Hopkins University. All documentation will be evaluated on a case-by-case basis, and students may be required to provide additional documentation and/or periodic reevaluation depending on the nature of the disability and documentation received. If a disability is a physical or sensory impairment of an unchanging nature, documentation does not need to be updated. Please note that sending documentation to the Student Affairs Office does not constitute a formal request for services. When the University's Director makes a determination about accommodations, the Peabody Office of Student Affairs will arrange an appointment with the student. Students must contact the Associate Dean for Student Affairs and Disability Support Services at the Peabody Conservatory, either in person or by telephone, to initiate the accommodation process and to discuss the implementation of reasonable accommodations.


Documentation

The guidelines about documentation of a disability have been established by the Johns Hopkins University Office of Institutional Equity, and are as follows:

  • Documentation must come from a qualified professional with relevant expertise in the disability in question. It must be signed by the diagnostician and printed on official letterhead.

  • Documentation must describe the current functional limitations resulting from a disability, and should include any relevant evaluations or diagnostic test scores.

  • Documentation must include a summary of the history of the disability.

  • Documentation should include a list of recommended accommodations that will address the functional limitations of the disability.

  • Documentation must refer to a recent diagnostic test or medical review, based on the type of disability.
